Shoulder pain can be split into two categories: joint-related and soft-tissue-related. The majority of joint-related causes include inflammatory processes in the joint, such as rheumatoid arthritis, osteoarthritis, and osteonecrosis, or joint trauma that causes cartilage injury. Shoulder joint discomfort can also be caused by shoulder instability, which occurs when the joint tries to dislocate, putting pressure on the joint component and surrounding soft tissue.

The shoulder or glenohumeral joint is the most mobile joint of the body and enables a wide range of motion that allows arm and hand to move in different directions and circular movements. The glenohumeral joint is a multiaxial synovial ball and socket joint and involves articulation between the glenoid cavity of the scapula (shoulder blade) and the head of the humerus (upper arm bone). The bones are held in place by ligaments, labrum and a group of four muscles and tendons called the rotator cuff

Athletes are especially susceptible to shoulder problems due to repetitive, intensive, inadequate training or traumatic injury:

  • shoulder instability
  • rotator cuff injury
  • Impingement syndrome
  • SLAP lesion
  • tendinopathies and bursitis
  • AC  joint lesions
  • Shoulder injuries can often be prevented, by following simple guidelines.

The Pre-season screening of athletes by the doctor and an experienced physiotherapist can help prevent common sports injuries. This should include assessment and management of :

  • sports-specific technique
  • flexibility and Core stability
  • rotator-cuff and scapular control
  • proprioception and general strength

Most cases of shoulder pain can be treated with conservative measures such as medication and physiotherapy, as well as arthroscopy of the shoulder or shoulder replacement surgery.
